In ancient times 1/3 of adult women died during child birth and that is just adult women... even today in cultures where 12 year girls are married off there is a very high mortality rate of young girls who get pregnant... their bodies are just not fully developed yet even if they have had their period and they haven't even stopped growing. Young girls who get pregnant not only face death and serious complications but even IF they do survive and the baby survives very often their bodies are torn and ruined... as the birth in underdeveloped girls can cause it to tear through their vagina, bladder and even intestines... so a lot of young girls who are married off at such a young age... IF they do survive they are incontinent for the rest of their lives, with urine and feces leaking because of being torn. Often in these cultures that still allow such early marriage of girls even if the girl survives the birth the man is disgusted that she then leaks piss and shit and they often beaten because of this.
